Enrollment in the Health Benefits for Immigrant Seniors (HBIS) program will be temporarily paused effective Nov. 6, 2023.

Due to repairs causing planned power outages in the area, the Joliet Regional Office located in the Bays Professional Center Building, will be closed to the public on Monday, May 6th.  The building is expected to reopen on Tuesday, May 7th.

CCBHC Model of Care and Overview

The SAMHSA CCBHC model ensures access to coordinated and comprehensive behavioral health care and aligns with Illinois’ commitment to person-centered treatment and optimization of the State’s behavioral health service delivery system. The Illinois CCBHC Demonstration Program will provide outpatient mental health (MH), substance use disorder (SUD), and primary care screening services, for all ages, regardless of their place of residence or ability to pay, via a common set of standards, tools, and organizational commitments to treat customers in a seamless and integrated fashion.

CCBHC Demonstration locations must meet rigorous established criteria for care coordination, service delivery, crisis response, and are evaluated by a common set of quality measures.

CCBHCs must provide 9 core services directly or through a designating collaborative organization (DCO). The 9 services are:

1. Crisis mental health services, including 24-hour mobile crisis teams, emergency crisis intervention services, and crisis stabilization.

2. Screening, assessment, and diagnosis, including risk assessment.

3. Customer-centered treatment planning or similar processes, including risk assessment and crisis planning.

4. Outpatient mental health and substance use services.

5. Outpatient clinic primary care screening and monitoring of key health indicators and health risk.

6. Targeted case management.

7. Psychiatric rehabilitation services.

8. Peer support and counselor services and family supports.

9. Intensive, community-based mental health care for members of the armed forces and veterans, particularly those members and veterans located in rural areas.

While CCBHCs are required to provide a complete scope of trauma-informed and recovery-oriented care to all individuals who present for treatment, Illinois has defined Illinois CCCBHC Service Requirements that each CCBHC must make available to ensure access to the most effective treatments.
