Update November 15, 2011, Meeting Notice

Due to the level of interest expressed, and after allocating space for committee members, presentation guests and required staff, we regret that we cannot confirm in-person attendance for the November 15, 2011, Care Coordination Subcommittee meeting. All confirmations to attend in-person are revoked. Attendance at both the Chicago and Springfield locations will be on a first-come, first-served basis. 

However, we will be offering a listen only Dial-In Option for the meeting. To listen to the meeting dial 1-877-402-9757 and use Participant Code: 8876267. Following the meeting, please feel free to submit your comments and questions to HFS.CareCoord@illinois.gov

If your interest in the meeting is mainly to keep informed on the development of the Innovations Project, we respectfully request that you use the Dial-In Option. This will allow space for those wanting to actively participate, and in particular give input on the performance and quality measures for the Innovations Project, to attend in-person.
