November 15, 2011, Meeting Notice

To members of the Care Coordination Subcommittee and others interested in care coordination:

As we stated at the Webinar and public meeting on the Innovations Project on October 13, all care coordination programs will be expected to meet certain quality and other performance measures.

At the next meeting of the Medicaid Advisory Committee (MAC)s Care Coordination Subcommittee, we will consider the HEDIS and HEDIS-like performance measures that will be included in the Innovations Project solicitation.  To the extent similar populations are being covered, the State will utilize uniform measures to compare the differing care coordination models.

The Subcommittee meeting will be a videoconference on Tuesday, November 15, from 10 a.m.-Noon at two locations: 401 S. Clinton, 1st Floor Training Room, Chicago and 201 S. Grand Avenue East, 1st Floor Large Conference Room, Springfield.  This will not be a Webinar, as this issue requires in-person discussion.  Space is limited, so if you are interested in attending, you must e-mail us at HFS.CareCoord@illinois.gov with the location you would like to attend.  We may not be able to accommodate everyone. If our room capacity is reached, you will receive an e-mail advising that we cannot confirm your attendance.   For those of you unable to attend, we will post notes from the meeting on the Care Coordination Web page at: http://www.illinois.gov/hfs/MedicalProviders/cc/Pages/default.aspx
