  3. Public Health/Vital Records Information/Birth Certificates – Birth certificate/record questions should be referred to the Illinois Department of Public Health (IDPH), Division of Vital Records at Birth Records (illinois.gov)

Documents, Brochures, and Forms Request

Healthcare and Family Services, Child Support Services supplies forms and brochures to hospital staff who assist parents in establishing parentage for their child.  Hospital staff may access  these forms on the Child Support Services Website, Brochures and Forms | HFS or order forms online at Request for Paternity/Child Support Services Forms | HFS.
