Pathways to Success is a program for Medicaid enrolled children under the age of 21 in Illinois who have complex behavioral health needs and require intensive home and community based services.  The program provides access to two tiers of care coordination including:

  • Tier 1 – High Fidelity Wraparound for children and youth with the most complex behavioral health needs.
  • Tier 2 – Intensive Care Coordination for children and youth with complex behavioral health needs.

The Pathways to Success program also covers additional services that can be provided in the youth's home and community including: Intensive Home-Based Services, Family Peer Support, Therapeutic Mentoring and Respite. To determine which youth are eligible for the Pathways to Success program and to ensure that they are receiving the most appropriate tier of care coordination, a Behavioral Health Decision Support Model is utilized. A description of the Behavioral Health Decision Support Model can be reviewed here.  

If you believe you or your child may be eligible for Pathways to Success, please submit a copy of the Request for Eligibility Determination form and a copy of your IM+CANS to the Bureau of Behavioral Health at the contact information on the form. Bureau of Behavioral Health will review your request and IM+CANS to determine eligibility for Pathways to Success.
