PACE of Southwest Chicago is a newly established PACE organization that will provide healthcare and social services to the older adults of West Chicago, Southwest Chicago, and nearby suburbs. The mission of the PACE of Southwest Chicago program is:

PACE of Southwest Chicago aims to ensure that every adult is – and feels – valued as they age. We offer integrated care, including a comprehensive suite of medical and social services, in a manner that promotes dignity, respect, and quality of life for the frail elderly population. Our organization supports the personal goals and desires of our participants, helping them thrive within their homes and communities, and within the broader community and family structures in which we operate.

PACE of Southwest Chicago will serve the following neighborhoods and cities: Austin, Bellwood, Berkeley, Berwyn, Broadview, Brookfield, East Village, Forest Park, Hillside, Humboldt Park, La Grange, La Grange Park, Lyons, Maywood, Melrose Park, Northlake, Oak Park, River Forest, Riverside, Stickney, Ukrainian Village, and Westchester.

We look forward to supporting seniors and the broader communities in the Chicagoland area, with services to commence in mid-2024.
