Federally Qualified Health Centers Cost Reports

All Federally Qualified Health Centers (FQHCs) participating in Medicaid must file a cost report. If a FQHC has more than one site, the FQHC has the option of combining the sites onto one cost report or filing separate cost reports for all sites. The cost report is due in the Bureau of Health Finance within 180 days after the close of the FQHC’s fiscal year. The FQHC must also submit audited financial statements along with the cost report.
