Family Case Management

All infants and women known to the department as being pregnant who are enrolled in the department's medical assistance programs, are referred to the Department of Human Services (DHS) for family case management services. HFS transmits the names of participants to Cornerstone, DHS's tracking system designed to track maternal and child health services provided by or through their provider networks.
