We cannot process any requests without a letter of representation on file. The letter of representation must meet the requirements listed below:
The Department is mandated to follow strict guidelines before releasing information to a custodial or non-custodial parent’s attorney.
The attorney representing a custodial parent or non-custodial parent must submit an individual request in writing printed on their letterhead.
The letter must meet the following requirements:
- Indicate the client you are representing, including a case identifier.
- State specific information being requested.
- Call back and fax number.
- The letter must be signed by the attorney.
We cannot honor a one-time generic request, or a request made on a fax cover sheet.
Please submit your request following the guidelines the Department is mandated to use.
