Application Agents
HFS and DHS Application Agents (AAs) help families to apply for Medical, Food and/or Cash assistance. AAs are community-based organizations, including faith-based organizations, day care centers, local governments, unions, medical providers and licensed insurance agents. An Application Agent Agreement with HFS or DHS allows agency staff to provide technical assistance in completing online and paper application and redetermination forms for individuals or families interested in receiving health, food or cash benefits AAs can also speak with DHS or HFS caseworkers as long as the State has the Application Agent Customer Authorization Form in the customers case records.
Application Agents can register to receive e-mail notification when new Application Agent Instructions and Alerts are posted to the Web site, by completing the Application Agents E-mail Notification Request.

Sec. 5. Healthcare program application assistance. It is a Class C misdemeanor for any person, including an individual, firm, corporation, association, partnership, joint venture, or any employee or agent thereof, to charge another person for assisting in completing and submitting an application for enrollment in any means-tested healthcare program administered by the Department of Healthcare and Family Services.
