The enrollment type definition is listed with each link to help you choose the proper steps to follow.

Please note the Modification Checklist has changed.  Providers can no longer end date an enrollment by answering that checklist question, as it has been removed.  Providers must upload a letter requesting an enrollment be end dated through the document upload feature within their IMPACT enrollment.
