Care Coordination Partner Organizations

1. How many separate partner organizations from one proposed Coordinated Care Entity (CCE) or Managed Care Community Network (MCCN) may request data?

One. This organization and its primary contact person should be named using the data request procedures listed below.

2. Can organizations other than potential Coordinated Care Entities (CCEs) and Managed Care Community Networks (MCCNs) access the data?

Not at this time. While we recognize that this data may be useful for a number of community health management and research purposes, this data is currently available only to potential CCEs and MCCNs who have filed a Letter of Intent (LOI), as described above.

3. What should I do if the primary contact or data contact changes?

Please inform Healthcare and Family Services by contacting the HFS contact identified in the cover sheet accompanying your data to share the name, phone number, and e-mail address of the new contact person. Additional documentation may be required.

4. What should I do if my organization/proposed Coordinated Care Entities (CCEs) and Managed Care Community Networks (MCCNs) would like to withdraw from Care Cordination Innovations Project (CCIP) bid consideration?

Please inform Healthcare and Family Services by contacting the HFS contact identified in the cover sheet accompanying your data.

5. What’s the difference between ‘primary contact person’ and ‘primary contact person for data’?

The primary contact is the person responsible for submitting the Letter of Intent (LOI), ensuring security and appropriate use of the data, and presumably overseeing the preparation of the proposal. The primary contact person for data (also known as the “data contact”) is the lead person for analyzing the data. Both roles can be performed by one person, but this is not required. The data contact may work for an organization other than the primary contact’s organization or be a third-party data consultant. (If the data contact is with another organization, that organization must be listed under Item 3a of the Data User Agreement.) The data contact will receive only the data-related communications. The primary contact will receive all communications regarding the solicitation, including copies of all data-related communications.
