Step 1: Survey

Step 2: Onboarding Package HFS sends to facility/organization

Once the survey is complete HFS will email the official onboarding package.  The package will included a link to the Discovery Form, link to the Clickwrap Form (MSA, BAA, SOF) and will verify your facilities status for IMPACT Terms and Conditions.  It also provides additional overviews regarding the program.

Step 3: Discovery Form

The discovery form helps to create an opportunity in the vendor’s software to help track your implementation and assist with long term success.

Step 4: Clickwrap Form (MSA, BAA, SOF)

Once the Discovery Form is complete PCC/CMT will review.  After approval your organization will receive a Click Wrap Agreement via email.  This will include the Master Service Agreement, Business Associate Agreement, and Service Order Form.  PointClickCare/Collective Medical will help answer any questions and provide a calendar to schedule a kick off call when the forms are complete.

Step 5: HFS IMPACT Terms and Conditions Accept updated T&C

The Onboarding Package will indicate if your facility needs to accept the update the HFS IMPACT Terms and Conditions.  These documents need to be accepted before live data can be exchanged on the platform.

Step 6: Implementation Starts Kick Off Call Scheduled

PointClickCare/Collective Medical will start the implementation process by scheduling a kickoff call with your facility.

       4. Complete and return clinical onboarding forms
       5. Schedule and complete platform training
       6. Training follow-up and optimization 
       7. Submit second file and sset to auto process
       8. Facility is Live and transition to support is initiated
