Information for Providers

​Under the Money Follows the Person federal demonstration grant, Illinois' Pathways to Community Living program provides individuals choices about where they can live, as well as assistance with community supports and services. Pathways is available to:

  1. Individuals who are over age 60,

  2. Individuals with physical disabilities,

  3. Persons with mental illness,

  4. Individuals with developmental/intellectual disabilities.

Trained transition coordinators from Centers for Independent Living, Case Coordination Units, Mental Health providers, DHS (Department of Human Services) Division of Developmental Disabilities’ Bureau of Transition Staff and other entities work under contract with the state and are responsible for the implementation of this program. They will come to your facility to identify and assess individuals who are interested and eligible for the program. Illinois has been transitioning individuals through this program since 2008, so you may already be familiar with transition coordinators and program representatives visiting your facilities.

Not everyone who is referred will be a candidate for transition. Living in the community requires both formal and informal supports, resources, and a desire to make it work. Long term care facilities will continue to have a role in providing medical and non-medical care to persons with physical and mental challenges. The Pathways program is focused on assisting those who are able to successfully live in the community.
