Information for Participants
The Pathways to Community Living program provides individuals choices about where they can live, as well as assistance with community supports and services. Pathways is available to:
Individuals who are over age 60,
Individuals with physical disabilities,
Persons with mental illness,
Individuals with developmental/intellectual disabilities.
The individual must also currently reside in a long term care facility (nursing home) or Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD).
To be considered for this program, an individual must:
Have resided in the facility for at least 90 days,
Be currently receiving or be eligible for Medicaid,
Must be interested in moving back to the community and must choose to do so,
Be able to have their needs met appropriately and safely in the community.
Individuals who meet these requirements may be eligible to participate. A trained transition coordinator can help you determine if you are eligible and if a community move is right for you.
If you meet the requirements of this program, your transition coordinator will provide one-on-one transition support services throughout the pre-transition process and for one year after you transition. Your transition coordinator will work with you to find qualified community housing, to create an individualized service plan based on your needs, and to assist with your move.
