Re:
Provider Revalidation Reminder – First Cycle Ending November 30, 2024

The Federal Centers for Medicare & Medicaid Services (CMS) requires all actively enrolled providers to revalidate or renew their Medicaid provider agreement at least every five years (42 CFR 455.414). The Department of Healthcare and Family Services (HFS) first informed providers of the revalidation process via the August 5, 2024, and August 28, 2024, provider notices.

The first revalidation cycle will be ending November 30, 2024. If you are part of this revalidation cycle and your revalidation is not submitted before the end of the cycle, you will be disenrolled, creating a break in your enrollment that will impact payments. Reactivation of your enrollment cannot be retroactive in accordance with federal regulations.

Providers are encouraged to review their revalidation cycle listed on the basic information page of their IMPACT enrollment and ensure the email listed for contact is up to date.

For additional information on the IMPACT system including frequently asked questions and other revalidation information, visit the IMPACT website.

Questions regarding this notice can be directed to the IMPACT Help Desk by calling 877-782-5565, option sequence 1, 2, 1.
