Thisprovider notice is being issued to alert subrecipients of the IllinoisDepartment of Healthcare and Family Services’ (HFS) CARES Program supported bythe Department of the Treasury Coronavirus Relief Fund (CRF) that the finalreporting template is available. 

 

Asstated in the provider notice dated January15, 2021, subrecipients have until March 29, 2021 to submit the finalreporting template and the supporting documentation.

 

Allunspent subaward funds must be returned to the Department at the followingaddress: Receipt Accounting at Illinois Department of Healthcare andFamily Services,

Bureauof Fiscal Operations, 2200 Churchill Road A-2, Springfield, Illinois 62702.

 

HFSCARES Final Reporting Template

Post-awardreporting will occur through the HFS CARES Portal. Additional details on thesubmission of your final report, the final reporting template and the templatefor the required cover letter can be found at the links below:

Round 1Subrecipients

HFSCARES Portal accounts were created for subrecipients who received a Round 1payment but did not register through the HFS CARES Portal. Accounts werecreated using the email address used by subrecipients to submit their executedsubaward agreement. Notification emails will be sent to these email addressesfrom the HFS CARES Portal, which will include instructions on how to completethe account registration and set your account password. Once completed,you will be able to modify or change the primary contact information andcontact email, if necessary. 

 

Round 1subrecipients who have not received an email notification from the HFS CARESPortal or need assistance to identify the email address associated with theiraccount should contact HFS for assistance. Call-in assistance is available from7 a.m. to 5 p.m. CST, Monday through Friday at 866-385-0600. Call centersupport is available in English and Spanish. Providers may also send questionsregarding the CARES payments via email to HFS.CARES@illinois.gov.

 

The CUREID of your subaward will be needed to allow our team to identify the emailaddress associated with your account. The CURE ID is the Unique Identifierfound on the first page of your subaward agreement.

 

Round 1subrecipients whose accounts require updates will still be able to access thefinal reporting template and required cover letter template through the linksidentified above and may begin preparing their report and gathering therequired supporting documentation.
