Illinois Child Support Application Download

Important: If you do not live in the State of Illinois, please contact the appropriate child support agency in your state for assistance.

The Child Support Application is available for download in Portable Document Format (PDF). It can then be filled out on your computer. If you don't already have Adobe® Reader, you will need to download and install it on your computer to view or print these files. Adobe® Reader is a free download from Adobe's Web site.
