To:       Skilled Nursing Facilities, Supportive Living Program Providers and Specialized Mental Health Rehabilitation Facilities

Re:      REMINDER - HealthChoice Illinois ADT Mandatory Participation

This notice reminds all skilled nursing facilities, supportive living program providers and specialized mental health rehabilitation facilities, hereinafter referred to as facilities/programs, that are enrolled in the IMPACT system and use an electronic health record (EHR) system capable of generating data in an HL7 format of their requirement to participate in HFS' statewide technology platform HealthChoice Illinois ADT.

The deadline for compliance with this requirement was December 31, 2021.

The Department is aware that some facilities/programs are not yet in compliance but are in the process of onboarding to the platform.

If your facility/program is not yet in compliance, you are required to complete the following survey to document your progress toward completing the onboarding process no later than January 26, 2022. The survey can be accessed through the following link:
