Re: Guidance to LongTerm Care Providers Regarding the Implementation of HealthChoice Illinois ADT

ThisNotice provides guidance to long-term care providers regarding theimplementation of HealthChoice Illinois ADT, the Illinois Department ofHealthcare and Family Services’ statewide technology platform to transmitelectronic messages from hospitals, skilled nursing providers and otherinstitutions and notify community providers of patient admissions, dischargesand transfers (ADT) to improve care coordination and quality.
Thefunctionality of HealthChoice Illinois ADT will support Illinois’ 2021-24Comprehensive Medical Programs Quality Strategy.
Allskilled nursing facilities, supportive living program providers and specializedmental health rehabilitation facilities that are enrolled in the IMPACT systemand use an electronic health record system capable of generating data in an HL7format will be required to transmit electronic notifications of admissions, dischargesand transfers for Medicaid customers to the HealthChoice Illinois ADT platformby December 31, 2021.
Illinoisprovider entities, including skilled nursing facilities, supportive livingprogram providers and specialized mental health rehabilitation facilities whoare enrolled in the IMPACT system and engaged in care coordination services forMedicaid customers will be eligible to subscribe to HealthChoice Illinois ADTto receive notifications from hospitals and long-term care facilities fortheir patients. It is anticipated that onboarding of subscribing providers,including long-term care facilities, will begin in August.
