Important Updates on the Medicare Medicaid Alignment Initiative (MMAI) transition to Dual Eligible Special Needs Plan (D-SNP)

This notice provides updates regarding the transition of the Illinois Medicare-Medicaid Alignment Initiative (MMAI) program to fully integrated dual-eligible special needs plans (FIDE SNPs) and the process for verifying FIDE SNP enrollment.

Department Enrollment Systems

***UPDATE:

The Department’s enrollment files are now updated with accurate FIDE SNP enrollment information and the instructions of Provider Notice issued 1/09/2026 no longer apply. The Department’s enrollment files include MEDI and 270/271 transactions. As stated in  Provider Notice issued 1/20/2026 and Provider Notice issued 12/24/2025, FIDE SNPs will appear in MEDI. The FIDE SNP information will be listed in both the “Medicare Advantage” and the “Managed Care Organization” details.  See MEDI screen print below: 

Instances where the Department’s enrollment file indicates the customer has FIDE SNP enrollment for Medicare but not for Medicaid is communicating that such customer is only receiving Medicare services from the FIDE SNP. In this scenario, the FIDE SNP contract information below will be listed in the “Medicare Advantage” details, and not in the “Managed Care Organization”. See MEDI screen print below:

In line with the Provider Notice issued 1/20/2026 and Provider Notice issued 12/24/2025, FIDE SNPs will appear as follows in MEDI:

Continuity of Care Requirements

As a reminder, the FIDE SNP health plans are required to offer the following continuity of care requirements:

• The FIDE SNP must offer a 90‑day initial transition period for Enrollees who are new to the D‑SNP. During this period, Enrollees may continue their current course of treatment with a Provider who is not part of the MCO’s network.

• The FIDE SNP must offer a 90‑day transition period for Enrollees transferring from another Health Plan to the MCO. This requirement applies to all Providers, including Behavioral Health Providers and LTSS Providers.

• The FIDE SNP must pay non‑network Providers for Covered Services delivered during the 90‑day transition period at the Illinois Medicaid Fee‑for‑Service (FFS) rate.
