Important Updates on the Medicare Medicaid Alignment Initiative (MMAI) transition to Dual Eligible Special Needs Plan (D-SNP)

This notice provides updates regarding the transition of the Illinois Medicare-Medicaid Alignment Initiative (MMAI) program to fully integrated dual eligible special needs plans (FIDE SNPs) and the process for verifying FIDE SNP enrollment.

January 1, 2026 Transition

The transition of the MMAI program to FIDE SNPs will be completed effective January 1, 2026.   Beginning January 1, 2026, dual members enrolled in the new program can receive health care services and prescription drugs through the FIDE SNPs.

***UPDATE: The Department’s enrollment systems are still in the process of being fully updated to reflect a member’s FIDE SNP enrollment. See below several options on how to verify if a member is enrolled in a FIDE SNP. The Department will issue an updated provider notice as soon as its systems are fully updated. 

Member ID Cards

The health plans have mailed member ID cards to all FIDE SNP members. Please ask FIDE SNP members to present their ID card at the time of service. The member ID card is used to identify FIDE SNP members but does not guarantee eligibility or benefits coverage because members may disenroll from a FIDE SNP or lose Medicaid eligibility at any time. Therefore, it is important to verify member eligibility prior to rendering any service.

Recipient Eligibility Verification System (REVS) accessed via the MEDI portal and the Internet Electronic Claims (IEC) web application (updated with plan name week of 12/29/2025; currently reflects only contract number)

Providers may verify enrollment in MEDI using each health plan’s Medicare contract number identified below.

Verifying Enrollment with a Health Plan 

Providers should use the health plan’s provider portal (available 24/7) as the first step to verify a member’ enrollment in a FIDE SNP.  Health plans will also have extended call center hours as indicated below.  See below specific information for each health plan.
