Updated IMPACT Terms and Conditions |
This notice informs providers of an update to the Department's Terms and Conditions document to which providers must attest upon submittal of an enrollment application, modification or revalidation. The Terms and Conditions apply to all Medicaid providers.
The updated Terms and Conditions are found on the IMPACT website at Terms and Conditions. A PDF document that identifies the changes in the Terms and Conditions is also available on the Terms and Conditions page.
Providers will be notified at the time they are to re-attest.
