Re:      DELAY in Reinstatement of the Certificate of Transportation Services (CTS) HFS 2271 Form - Now Effective June 1, 2022

_______________________________________________________________________________________________________________________________________________________
This notice informs providers of a delay in implementing the use of the Certificate of Transportation Services (CTS) form (HFS 2271) for all non-emergency transports originating at a customer's private residence.

By informational notice dated April 22, 2022, HFS notified providers that the HFS 2271 form will be used for prior and post approval purposes to determine the appropriate level of medically necessary non-emergency transports originating at a customer's private residence. This applies to customers covered under the Medicaid fee-for-service (FFS) plan, as well as HealthChoice Illinois (HCI) managed care plans and the Medicare/Medicaid Alignment Initiative (MMAI) plans.

The effective date of implementation for the HFS 2271 form has been delayed until June 1, 2022, to allow providers and customers additional time to prepare for the requirement. HFS will also post a question-and-answer document in the near future to address inquiries that have been received.

Questions regarding this notice may be directed to the Bureau of Professional and Ancillary Services at 877-782-5565 for fee-for-service Medicaid participants, or to the applicable HCI or MMAI managed care plan.
