Re:      Integrated Health Homes Town Hall Meetings

Please join us for a one of the upcoming Town Hall style discussions on our upcoming Integrated Health Home (IHH) models (one for children and one for adults) to significantly enhance care coordination for Medicaid members across their physical, behavioral and social care needs.

 

Two events have been scheduled where interested parties may participate in person.  Additional town hall meetings will be scheduled in the near future.
