Re: Colbert v. Quinn (07 C 4737) Implementation Plan

An Informational Notice was sent on February 19, 2013, Provider Notice, Colbert v. Quinn (07 C 4737) Implementation Plan, informing you of the processes to engage and evaluate Colbert Class Members residing in nursing facilities in Cook County.

Effective January 22, 2014, oversight of the Colbert Consent Decree Implementation has been transferred from the Healthcare and Family Services to the Department on Aging (DoA). While DoA begins to assume this responsibility, we want to inform you of one change that is planned to begin as soon as possible.

Clinical staff from Lutheran Social Services of Illinois (LSSI) and Metropolitan Family Services (MFS) will be conducting reassessments and Resident Reviews on a select group of Colbert Class Members who have been diagnosed with a serious mental illness. Many of these nursing facility residents have been, and will continue, to work with Aetna Better Health or IlliniCare.

Staff from LSSI and MFS will be in contact with your facility to alert you when they will make their visits and what nursing facility residents they will be interviewing. Staff will present identification to facility personnel once they arrive.

Please remind your staff of the important role they play in facilitating access to nursing facility residents and their records, including all medical records.

If you have any questions, please contact DoA, 312-814-2630.
