Tailored Care Management Program for Williams and Colbert Class Members

What are the Williams and Colbert consent decrees?

Through the Williams and Colbert Consent Decrees, the State of Illinois agreed to provide Class Members with supports and services to help Class Members transition to and live in the community, in the most integrated setting for their needs.  

In the Williams Consent Decree, Class Members are adults with serious mental illness who have been living in Specialized Mental Health Rehabilitation Facilities (SMHRF) but whose needs can be served in the community. The State provides services and supports to Williams Class Members who wish to move out of the SMHRF into a community-integrated setting.  

In the Colbert Consent Decree, Class Members are Medicaid-eligible adults with disabilities who have been living in Cook County nursing facilities but whose needs can be served in the community. The State provides services and supports to Colbert Class Members who want to move out of the nursing facility and live in a community setting.

How is HFS involved with the consent decrees?

The Illinois Department of Healthcare and Family Services (HFS) is a co-defendant in the Williams and Colbert Consent Decrees and works closely with the Department of Human Services (DHS) and other State agencies to serve Class Members. HFS operates the new Centralized Outreach and Assessment program and is working closely with DHS to design a new Tailored Care Management Program for Class Members.  

What is HFS doing to improve the class member experience?

HFS’s responsibilities for providing services to Williams and Colbert Class Members have expanded in recent years. Through HFS’s Community Transition Initiative (CTI), Medicaid Managed Care Organizations (MCOs) provide services and supports to many of their members who wish to transition to the community. In December 2024, HFS launched a centralized program to provide outreach and assessments to Williams and Colbert Class Members living in SMHRFs and Cook County Nursing Facilities.

The current structure of the State’s existing Consent Decree transition services – spanning two programs and more than a dozen contractors – has allowed best practices to emerge for Class Members wishing to transition to the community. To ensure scalability and improve quality, the State plans to implement a Tailored Care Management Program to deliver consistent and integrated services to Class Members who wish to live in the community.

HFS is actively seeking feedback from the public on their experiences with current Williams and Colbert programs and services and where they see opportunities for improvement.
