Tips for Telling Us Your New Address

If  you get cash assistance or SNAP (food stamps) or medical benefits like Medicaid or All Kids from the state of Illinois, you must be sure we always have your correct address.

You can change your mailing address online with U.S. Postal Service or in-person at your  U.S. Post Office. If you keep your address updated with the U.S. Post Office, then your change of address will be reported automatically to Healthcare and Family Services and the Department of Human Services. That way we will automatically change your address for cash assistance, SNAP (food stamps) or Medicaid and All Kids.

Or if you have an All Kids Share or All Kids Premium case, you can tell us your new address by filling out a form online at State of Illinois All Kids

Information you will need when calling about SNAP or Cash Assistance case:

  • The full 9-digit social security number
  • Date of birth
  • 19-digit number printed on the front of your Illinois Link card, or your 4-digit PIN

When you call the Link Help Line, if you do not have your card number, do not enter anything and wait until you are asked if you would like to use your social security number, date of birth and PIN. 

The social security number and date of birth are usually for the primary card holder, but may be for someone else in the case.
