2024 Long Term Care (LTC) Cost Report Due Date |
This notice informs long term care (LTC) providers that the Department of Healthcare and Family Services (HFS) has adopted amendments effective March 27, 2025 to 89 Ill. Admin. Code sections 140.543 and 140.560 concerning the annual filing deadlines for Long Term Care (LTC) facility financial and statistical reports (cost reports).
The rulemaking extends the cost report due date for facilities whose fiscal year ends on or before June 30 to November 30 of the applicable year. For facilities whose fiscal year ends after June 30, the report is due 5 months after the fiscal year end date (previously, all reports were due within 90 days after the end of the facility's fiscal year.)
For the current 2024 LTC cost reporting year, the December 31, 2024, cost reports will be due May 31, 2025.
The due date for the Supportive Living Provider (SLP) cost reports has not changed.
The 2024 LTC cost report files are available to download from the HFS Cost Reports webpage.
Upon completion of the 2024 LTC cost report file, send by email or CD to
HFS.HealthFinance@illinois.gov. A signed paper copy must be sent in also to:

As a reminder, when completing the LTC cost report, please ensure that the Ownership Listing pages are fully completed and include 100% ownership of the facility and the column for the owner’s middle initial is also completed for each owner.
