 Update: Publication of the Certified Community Behavioral Health Clinic Learning Collaboration Application for Demonstration Year 2 

This notice informs providers of an update to the March 7, 2025 provider notice: Publication of the Certified Community Behavioral Health Clinic Learning Collaboration Application for Demonstration Year 2.

HFS is extending the due date for the CCBHC Learning Collaborative Application to 11:59 p.m. on April 18, 2025.

To take part in the Learning Collaborative, prospective CCBHC sites must apply and be accepted for participation. The Learning Collaborative Application is hosted through an online Qualtrix Survey. The application for participation in the Learning Collaborative and supplementary materials are available at the following link: https://hfs.illinois.gov/medicalproviders/certifiedcommunitybasedhealthcenterinitiative.html.
