 HFS Family Planning Program – Allow Signature of Minor on Application

This notice confirms to providers that minors may sign their own applications for the Department of Healthcare and Family Services’ (HFS) Family Planning Program.

Per Public Act 103-0786, a minor may sign and file an application on the minor's own behalf if the application is for the 305 ILCS Article V family planning program established under Section 5-5 by Public Act 102-665.

Further information on covered services and how to apply for the HFS Family Planning Program is available in provider notices dated August 2, 2023 and August 31, 2023, as well as on the HFS website.
