To:       Long Term Care (LTC) Facilities - Nursing Facilities (NF), Supportive Living Program (SLP), and Specialized Mental Health Rehabilitation Facilities (SMHRF)

Re:      Patient Credit File Layout Update    _______________________________________________________________________________________________________________________________________________________

This notice informs Long Term Care (LTC) providers that effective the weekend of April 6, 2024, the LTC Patient Credit File (PCF) layout was updated to reflect two new fields: the hospice effective date and the hospice end date. An example of the LTC Patient Credit File Layout is linked below.

Patient credit files are created on a weekly basis. Instructions on how to download the provider’s patient credit file from the Medical Electronic Data Interchange (MEDI) System can be found in the Provider Notice Issued 04/11/2019 (illinois.gov).
