Re: Implementation of HealthChoice Illinois Admission,Discharge and Transfer (ADT) Platform
Thisnotice provides guidance to ambulatory/community providers regarding theimplementation of HealthChoice Illinois ADT, HFS’ statewide technology platform,to facilitate transmission of HL7 formatted messages from hospitals, skillednursing facilities and other institutions and notify ambulatory/communityproviders of patient admissions, discharges and transfers to improve carecoordination and quality of care.
Thefunctionality of this technology platform will support Illinois’ 2021-24Comprehensive Medical Programs Quality Strategy.
Illinoisproviders enrolled in the IMPACT system and engaged in care coordinationservices for Medicaid customers will be eligible to subscribe to HealthChoiceIllinois ADT to receive ADT notifications for their patients. It isanticipated that onboarding of ambulatory/community providers will begin inAugust or September 2021. Onboarding packets were sent to the Department’smanaged care plans on May 27, 2021. Additional information on the impact to themanaged care plans will be provided at a later date.
Ambulatory/communityproviders are asked to complete a short survey to assist HFS in determiningprovider readiness and technology resources. Please access the survey at: https://www.surveymonkey.com/r/DGJQDZB. Providersare requested to complete the survey by Friday, July 9, 2021.
HFS willhost two online demonstrations of the technology platform for providers engagedin care coordination. The content of both sessions will be the same. Interestedproviders are encouraged to attend.
