Re:      ​Letters of Intent for Upcoming HealthcareTransformation Collaboratives Funding Due June 15, 2021

 

 

This notice is a reminder that entities intending to apply for upcomingHealthcare Transformation Collaboratives funding can still formally notify theIllinois Department of Healthcare and Family Services of their intention.

 

Entities are strongly encouraged to file a Letter of Intent if they areplanning to apply for funding during State Fiscal Year 2022 (July 1, 2021 –June 30, 2022). The Department of Healthcare & Family Services plansto initiate a new round of funding in September 2021.  Letters for thoseseeking funding during this cycle are to be received no later than June 15,2021.

About HealthcareTransformation Collaboratives

 

The Healthcare Transformation Collaboratives program makes available asmuch as $150 million per fiscal year beginning in State Fiscal Year 2021. Thisfunding is available to collaborations between care providers, includingpreventative care, primary care specialty care, hospital services, mentalhealth and substance abuse services, and community-based entities that addressthe social determinants of health. Priority will be given to collaborationsthat include safety net hospitals or critical access hospitals.

 

The application period for the first round of awards has closed; however,a second round of applications will be available for the fall.
