Re:​ Revised Alpha and Numeric Lists of Pharmaceutical Labelers with Signed Rebate Agreements

Attached are the most current alpha and numeric lists for pharmaceutical labelers having signed Medicaid Drug Rebate Agreements on file with the Centers for Medicare and Medicaid Services (CMS).

Providers are reminded that the rebate program applies only to drugs as defined in the Food, Drug and Cosmetic Act. Covered non‑drug items such as needles, syringes, diabetic testing supplies, lancets and ostomy items may be dispensed irrespective of the labeler, whether or not the labeler has signed a rebate agreement with the federal government. When preparing a claim, the pharmacy must always use the accurate eleven-digit NDC number of the products being dispensed.

Pharmacies will be expected, on audit, to produce invoices reflecting the pharmacy's purchase of drug products billed to the department, irrespective of the pharmacy's source of the drug products' acquisition.
