Re: Removal of Certain Prior Approval Requirements and Changes to Quantity Limits

 

The department recently reviewed its prior approval policy for certain durable medical equipment and supplies, and orthotic and prosthetic equipment and components.  As a result, effective with dates of service on and after November 1, 2008, the department will make the following changes:

  • The department will remove the prior approval requirement on 13 durable medical equipment and supplies codes.
  • The department will remove the prior approval requirement on 125 orthotic and prosthetic equipment and components codes.
  • Quantity limits will be increased on 14 supply item codes. 

The affected HCPCS codes are identified in a separate “Changes from Previous Posted Listing” document, on the Web site at:

These code changes have also been incorporated into the revised DME Fee Schedule, on the Web site at the link shown above.
