Except as indicated below, 89 Illinois Administrative Code 140.513, Notification of Change in Resident Status, requires all long term care facilities to report a resident’s death within five working days after death by sending a telefax transmittal on Form HFS 1156, Long Term Care Notification, to the Department of Human Services (DHS), Exception Processing Unit. The facility must maintain a copy of Form HFS 1156, and the fax confirmation sheet as evidence of timely submission. The dedicated fax number for notification of a resident’s death is 217-557-4210. Do not use any other fax number for this notification.
Facilities that participate in the Recipient Eligibility Verification (REV) System do not have to fax this information. However, they must electronically submit notification to the Department of any change in a resident's status through their REV vendor within five working days after the change.
Additionally, except for facilities that participate in REV, a facility must send an original copy of the notification of death, discharge or any other changes in a resident's status, on Form HFS 1156, to the facility's DHS local office caseworker within five working days after the change.
Facilities with the highest incidences of overpayments due to untimely notification of deaths or discharges will be required by the Department to participate in the REV System. For any such facility, this requirement may be removed by the Department after one year of compliance.
