Effective immediately, the Department of Human Services, Division of Mental Health, (DHS/DMH) will begin implementation of targeted resident review activities for residents of nursing facilities with serious mental illness (SMI). These activities will be conducted through local agencies that have contracts with DHS /DMH for the provision of Mental Health Preadmission Screening and Resident Review (MH PASRR). The local MH PASRR agencies should be familiar to nursing facilities, since these agencies already perform the mandatory screening prior to nursing facility admission. Nursing facilities will be contacted by their local MH PASRR agencies. Initially, resident review activities will be conducted in the counties of Cook, Lake, Will, McHenry, Kane, Kendall, Grundy, and Kankakee. Statewide implementation will begin by State Fiscal Year 2008 (July 1, 2007).
The initial phase of resident review implementation will focus on the following groups: (1) new admission of individuals with SMI to nursing facilities (not transfers or readmissions after acute care); (2) residents with SMI who have had a significant change in condition; and (3) other possible clusters of residents with SMI identified by DHS/DMH. Nursing facility residents will be identified for resident review by DHS/DMH, which will transmit the names to local MH PASRR agencies.
To conduct resident review, MH PASRR agencies shall have timely and unfettered access to the identified resident, to the resident’s nursing facility records, and to nursing facility staff that are part of the resident’s treatment team. The review will include assessment, continuing care needs, the potential for community reintegration within six months, and any recommendations regarding services in the nursing facility treatment plan. MH PASRR agencies will receive instructions from DHS/DMH for sharing the results of the resident review process with the nursing facility and for working through any issues or disagreements regarding the results.
Residents who are identified as having the potential for moving to a more independent setting in the community within six months, and who choose to work toward transition will be referred to DHS/DMH for prior authorization of Targeted Case Management (TCM) services. TCM services will be provided through MH PASRR agencies and will include the development of a transition plan, the identification of housing options, the establishment of linkages with necessary community services prior to discharge, and other activities necessary to ensure successful community reintegration. This resource, which has not been available previously, should provide welcome assistance to nursing facilities.
