Re: Cost Reports

 

Recently adopted amendments to the Supportive Living Program rules require all supportive living facilities (SLFs) to submit cost reports at any time upon request by the Department or when a significant change occurs in the SLF's financial status/solvency, and annually not later than 90 days after the end of the SLF's fiscal year (89 Ill. Adm. Code 146.265(c)(5)). This requirement replaces the previous rule requirement of annual financial statements.

The cost report and instructions may be downloaded from the Department's SLF Web site. For the 2004 cost report year, the due date will be extended due to the fact that this is the first year a cost report is being required. All SLF providers are expected to complete, and submit, a 2004 cost report to the Department no later than September 30, 2005. When filing the cost report be sure to submit a signed paper copy along with a copy of the file on 3.5" disk, CD or by E-mail. You may send the cost report by E-mail to HFS Webmaster.

Each enrolled SLF shall file an annual report with the Department in accordance with the following requirements:

  • All schedules contained in the cost report must be completed with the exception of those schedules specified in the cost report instructions as optional.
  • The cost report is not complete until all required schedules are filed and all inquiries to the provider are satisfactorily resolved.
  • If the cost report is prepared by someone other than the facility's manager or officer, the certification must be signed by the preparer as well as the officer or manager. The preparer's declaration is based upon all information of which the preparer has any knowledge.
  • All financial data contained in the costs report must be accounted for on the accrual basis of accounting.
  • Copies of all independent audits and reviews performed on the SLF by certified public accounting firms shall be provided to the Department with the cost report.

 

For SLFs that have undergone a change of ownership, the new owner or lessee must file a cost report nine months after acquisition (covering the first six months of operation). A change of ownership is dated from the closing of the sale or from the date of the oldest lease agreement between the present incumbents of a lease. The facility must also file a cost report within 90days after the close of its first complete fiscal year. A change of corporate stock ownership does not constitute a change of ownership.

 

For new SLFs certified by the Department, a full cost report must be filed within nine months after opening the facility (covering at least the first six months of operation). The facility must also file a cost report within 90 days after the close of its first complete fiscal year.

 

One extension up to 30 days shall be granted for circumstances that will not allow a cost report to be properly completed before the due date of the report. The written extension must be submitted to the Department's Bureau of Long Term Care prior to the original due date.

 

SLFs that fail to file an annual cost report risk having their payments held by the Department until a cost report is submitted (89 Ill. Adm. Code 146.265(d)).

                                                             Public Aid has a new name.

The name of the Department of Public Aid (DPA) has changed to Healthcare and Family Services, HFS for short. 

 

You will soon see the new name on forms, letters and the Internet. Until existing form supplies are exhausted, however, we will continue to use materials bearing the former DPA name and logo. Either version of documents should be considered official regardless of the department name they bear.

 

Only our name has changed. HFS's programs, staff, office locations, mailing addresses and phone numbers remain the same as DPA's.

 

We believe our new name more clearly reflects our mission of service to the State of Illinois including providing access to health care for over 2 million Illinoisans. In our work, we deeply appreciate your participation in our programs. We hope this change poses no inconvenience or confusion for you.
