Re:     Submittal of Medicaid Eligibility Redetermination Form

The Department has become aware of an issue with the fax number listed in the contact information on the Medical Benefit Renewal Notice for the annual redetermination (REDE) of Medicaid eligibility which does not allow the successful submission of documentation.  Until the issue is resolved, providers should use the mailing address or fax number of the appropriate Department of Human Services (DHS) Medical Field Operations (MFO) office to submit all REDE forms and supporting verifications.
