ICF/IID and MC/DD Rate Changes Effective January 1, 2026

This notice informs Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IIDs), Medically Complex for the Developmentally Disabled Facilities (MC/DDs), and associated Developmental Training providers that the Department of Healthcare and Family Services (HFS) has received federal approval to implement the rate increases specified in Public Act 104-0002. For purposes of this provider notice, Developmental Training and Community Day Services have the same meaning such that Developmental Training providers and settings are the same as Community Day Services providers and settings. 

Rate increases include:

• an $0.80 per hour wage increase for all direct support personnel and all other frontline personnel who are not subject to the Bureau of Labor Statistics' average wage increases, who work in residential and community day services settings, with at least $0.60 of those funds to be provided as a direct increase to all aide base wages, with the remaining $0.20 to be used flexibly for base wage increases to the rate methodology for aides.

• an increase sufficient to provide wages for all residential non-executive direct care staff, excluding aides, at the federal Department of Labor, Bureau of Labor Statistics’ average wage.

• exceptional care rates for MC/DD facilities will be increased in an equivalent manner as the base rates.

January 1, 2026 rates have been entered into the State’s payment system. HFS will systematically reprice claims that were paid at lower rates. Vouchers for the payment adjustments will be generated and sent to the Comptroller in late August or early September 2026. All new claims are being priced using the updated rates.

Provider specific per diem rates for ICF/IID and MC/DD providers can be found on the HFS website under Medicaid Reimbursements. In addition to the per diem rates, files are included for developmental training rates.

Download Rate Sheets through the Medical Electronic Data Interchange (MEDI):
Individual providers can download rates sheets through the MEDI portal using the following instructions:

1. Login to MEDI 
2. Select Internet Electronic Claims (IEC) 
3. Select Download File(s) 
4. Choose the “Entity” from the drop-down box 
5. Select the file you wish to download from the “Available Files” listed 
6. Enter a “Local Directory Location” (a folder created on your computer or network) to accept the downloaded file 
7. Click “Download Files” button 
8. Go to your designated directory location 
9. Open the downloaded file (recommended program is WordPad)

Please note that the rate increases and subsequent payment adjustments include developmental training service rates, as well. The payment adjustments for these services will need to be passed on to the appropriate developmental training provider within 10 days as specified in administrative rule at 89 Ill. Admin. Code 140.646(d).
