The Voluntary Acknowledgment of Parentage (VAP) form(s) can be digitally submitted by completing the information below and attaching the documents. Multiple VAPs can be submitted at the same time by attaching separate PDF documents or a PDF ZIP file.

Please attach the completed VAP(s) and/or accompanying VAP Documents in PDF format or complete an electronic VAP by clicking HERE. Once the electronic VAP has been completed, save the document as a PDF. Open the PDF document and complete the electronic signature of both parents and witnesses, and save your updated PDF. Lastly, attach the VAP(s) below.
