This notice is to inform all Supportive Living Program (SLP) providers that in order to ensure accurate monitoring and appropriate processing of admissions when an individual transitions from the conventional side of a SLP to the Dementia Care Unit (DCU) of a SLP, the following steps must be followed:
1). The provider needs to enter the discharge of the conventional SLP in MEDI and then submit a new admission Transition Audit Number (TAN) in MEDI for the SLP DCU. The admission should automatically be added the next day.
2). To ensure there are no gaps in service/claims, the discharge of the previous category of service (COS) admission needs to be the same date as the new admission. The discharge destination should be coded as a transfer to a new facility.
Example:
The resident was initially admitted to the Woodridge conventional SLP on 04/01/2025. Then the resident transitioned to the DCU at Woodridge on 06/02/2026.
