 IM+CANS Learning Collaboratives 

The Department of Healthcare and Family Services (HFS), in partnership with the Provider Assistance and Training Hub (PATH), is excited to announce the launch of Learning Collaboratives for the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS).

The Learning Collaboratives will be provider-driven conversations regarding the use of the IM+CANS in the provider community, best practices, and how the IM+CANS should drive collaboration between providers. The Learning Collaboratives are designed to support providers through shared learning and continuous improvement in the delivery of services and implementation of the IM+CANS. Both in-person and virtual options will be offered, facilitated by PATH.

Informational Webinar

HFS is hosting an informational webinar regarding the Learning Collaboratives on June 3, 2025, at 2:00pm. The webinar will overview the purpose of the Learning Collaboratives and provide an opportunity to meet the PATH team who will facilitate the Learning Collaboratives.
