​Re: Email Addresses for Optometric Prior Approval Requests and Eyeglasses Status Checks

This notice informs providers that effective March 1, 2024, the Department of Healthcare and Family Services (HFS) will no longer accept optometric prior approval requests or eyeglasses status checks submitted via fax. This affects requests only for the Medicaid fee-for-service program.

HFS has created two new emails for providers to submit those requests. Providers may begin using these email addresses immediately:

·Optometric prior approval requests - HFS.Opticalpriorapproval@illinois.gov
·Eyeglasses status checks - HFS.AdultGlassesEligibility@illinois.gov

Prior approval requests still being faxed beginning March 1, 2024, cannot be processed, and no response will be given to faxed requests.
