​Re: Specialized Mental Health Rehabilitation Facilities and Cook County Nursing Facilities PathTracker Monthly Census Requirements

This notice serves as a reminder that pursuant to 77 Ill. Adm. Code Part 300 and 77 Ill. Adm. Code 380,Specialized Mental Health Rehabilitation Facilities (SMHRFs) are mandated to submit a census report to the Illinois Department of Human Services (DHS) on a monthly basis. The census report must reflect an accurate census of all Medicaid-eligible residents and the previous month's voluntary and involuntary discharges conducted under   77 Ill. Adm. Code Section 300.3300 including any voluntary and involuntary discharges

scheduled to be conducted within 48 hours after the end of the reporting month. This monthly census report is required to be entered into PathTracker, regularly, and must be found in compliance by the fifth (5th) business day of every month. As of June 1, 2023, the entire facility’s census, including new admissions and any relevant discharges of residents already listed in the facility’s census (if applicable), must be entered using PathTracker.

The Illinois Department of Public Health (IDPH), in partnership with the Illinois Department of Healthcare and Family Services (IDHFS), launched the use of PathTracker to maintain census and document admissions and discharge. Both agencies hosted a mandatory attendance session to introduce the PathTracker system and educate facility staff on how to utilize the system for timely and accurate reports. As of June 1, 2023, PathTracker will be the only system through which your facility will do the required census reporting, by ensuring that all current and future residents are entered into the PathTracker system upon admission and updated at time of discharge to comply with the requirement of the Administrative Code. The census report is not longer required to be submitted to DHS.

The first (1st) submission in PathTracker will require the entry of all existing Medicaid-eligible residents not already in PathTracker (i.e., all residents who were admitted prior to the new Preadmission Screening and Resident Review (PASRR)/SMHRF preadmission processes) and currently on your census.

The Medicaid-eligible resident’s nine (9) digit Medicaid Recipient Identification Number (RIN) must be entered into PathTracker when adding residents to the census or when the existing census is updated.

Failure to submit timely and complete census reports will result in licensure penalties pursuant to IDPH enforcement authority.
