Re:      Address Change for Remitting Monies Remaining in Deceased Patient Accounts

This notice informs nursing facilities of a new address to remit monies remaining in deceased patient accounts that may be owed to the Department of Healthcare and Family Services (HFS).

Effective immediately, please use the following address to remit payments to HFS:
