Re:     Private Ambulance American Rescue Plan Act Recovery Funds

The Illinois Department of Healthcare and Family Services (HFS) is conducting a survey to obtain information for the development of a grant program to distribute American Rescue Plan Act (ARPA) State and Local Fiscal Recovery Funds to private ambulance service providers.

Private ambulance service providers are asked to click on the link Ambulance Provider Information Request and complete the survey no later than March 8, 2023.
