This notice informs providers of the qualified Care Coordination and Support Organizations (CCSOs) in Designated Service Areas for the Pathways to Success program.

The list of qualified CCSOs can be found at the Resources link. Additional information is available on the Pathways to Success program main webpage.

Questions regarding this notice may be directed to the Bureau of Behavioral Health via email at HFS.BHCompliance@illinois.gov.

Billing questions may be directed to a billing consultant in the Bureau of Professional and Ancillary Services at 877-782-5565 for fee-for-service claims, or the applicable HealthChoice Illinois managed care organization plan.
