Re: Letters of Intent for Upcoming Healthcare Transformation Collaboratives Funding Now Being Accepted
Please be aware that entities intending to apply for upcoming HealthcareTransformation Collaboratives funding may now formally notify the IllinoisDepartment of Healthcare and Family Services of their intention.
Entities are strongly encouraged to file a Letter of Intent if they areplanning to apply for funding in Fiscal Year 2022 or future cycles. Letters forthose seeking funding in this fall’s cycle are to be received no later than June15, 2021.
Instructions and the form to complete a Letter of Intent may be found atthe Healthcare TransformationCollaboratives website under "Applications and Tools.
About HealthcareTransformation Collaborative
The Healthcare Transformation Collaboratives program makes available asmuch as $150 million per fiscal year beginning in State Fiscal Year 2021. Thisfunding is available to collaborations between care providers, includingpreventative care, primary care, specialty care, hospital services, mentalhealth and substance abuse services, and community-based entities that addressthe social determinants of health. Priority will be given to collaborationsthat include safety net hospitals or critical access hospitals.
The application period for the first round of awards has closed; however,a second round of applications will be available for the fall.
